In what cases could microscopic root canal treatment provide solutions?

Microscopic root canal treatment step by step

1. In order to explore and analyse root canals, x-rays (if necessary, also CT scans) are taken of the given tooth.

2. Anaesthesia - in order to ensure a completely pain-free treatment

3. The complete isolation of the tooth (placing a rubber dam) in order to establish a germ free environment.

4. Exploration of the tooth structure with the least possible drilling in order to reach root canals.

5. Cleaning / sterilisation of the root system or removing the old root filling. If necessary, we close the tooth with a temporary, medicated filling.

6. Root length measurement using a special electrical device.

7. Closure of the root canals - root filling

8. Making a cover filling / inlay / onlay or a crown.

What’s the difference between the traditional and the microscopic root canal treatment? 

The largest benefit of microscopic root canal treatment is the magnifying function of the dental microscope which enables the dentist to examine the tooth and its root canals under multiple magnification (even 30x). This way, those areas that are invisible to the naked eye can be explored and treated as well, which greatly increases the success and the efficiency of the treatment. Whether we’re talking about the manual or the mechanical root cleaning method, the whole intervention becomes remarkably accurate. Even root canal filling can be done more precisely.

Coaxial illumination: the special LED light built inside the head of the microscope illuminates exactly the surface to be treated. This direct illumination provides excellent visibility for the dentist to examine all the details of the problematic area. 

When would it be necessary to have a microscopic root canal treatment?

If pain persists following the traditional root canal treatment and your tooth continues to cause complaints, it’s highly possible that there are unexplored, uncleaned / unsterilised root canals left. Such root canals are usually invisible to the naked eye. With the help of microscopic magnification, it’s possible to discover and clean hidden / curved root canals.

The treatment (as in the case of traditional root canal therapy) is carried out under anaesthesia - thus, it’s completely painless. Tooth sensitivity or pain may occur up to 2-3 weeks after treatment, which is considered to be completely natural.

There is no treatment providing a 100% result, since several parameters influence the recovery process. However, microscopic root canal treatment provides an outstanding result with its 99% success rate compared to the traditional root canal therapy.

Possibly, because it could be more time consuming. On the other hand, using a microscope does not make a difference in other aspects: for example the application of rubber dam is usual in all kind of root canal therapies.

Microscopic root canal therapy is one of the most precise endodontic interventions. Logically, it’s more time-consuming than a traditional treatment - thus, a single occasion may take up to 1,5-2 hours. However, we would note that usually only 2 (in rare cases 3) interventions are necessary to finish this kind of microscopic, high precision root canal therapy. In the past, dentists had to change the medicines inside the treated tooth even 5-7 times.

Before installing the rubber dam, we place a lip / cheek retractor on the opposite side of the treated area (if the patient requires). It’s an instrument that the patient can bite on - thus, joints and chewing muscles are subject to minimal load during the intervention. 

  • In order to carry out such interventions, high-value equipment is required. It’s a significant investment from the dentistry’s side - contrary to those clinics where this more efficient kind of therapy is not available.
  • This kind of treatment demands special tools, materials and instruments (high-tech apex locator, nickel titanium instruments that can be used during a limited number of occasions, machine needles, specialty pharmaceuticals, etc.)
  • A root canal therapy that is carried out using a microscope and the latest, modern technology is more time-consuming than the traditional one.
  • The intervention is done by a clinical endodontist with highly specialised expertise.

As far as the price is concerned, we would like to point out that the given tooth need to be extracted if the root canal treatment is unsuccessful. Substituting an extracted tooth with an implant may be a much more costly intervention than the microscopic root canal treatment itself, providing that bone graft + implantation + superstructure + crown might be needed at the same time. What is more, since each recovery phase builds on the last, the complete time commitment is expected to be longer. The removed tooth may be replaced with a bridge as well, but it’s not a negligible aspect that the adjacent (in many cases, healthy) teeth must be polished.

Therefore, we believe that everybody must save their own teeth by all means!
